Connecting Residential Treatment with Continuing Care at Home
Applied Psychology's My Rehab Link helps Australians returning from overseas residential alcohol and drug treatment establish appropriate continuing care before they leave rehab.
Working alongside the rehab’s existing discharge planning and aftercare, we help ensure the receiving-care connection is in place with a clinician identified, an introduction completed and the first appointment arranged for the client’s return to Australia.
How My Rehab Link Works
The process begins before discharge and follows six simple steps:
Plan Before Returning Home
The rehab and client identify that ongoing support in Australia would be helpful and connect with Applied Psychology before discharge.
Understand What the Client is Returning To
The client completes a brief return-home form covering their concerns, existing supports, home circumstances and what they would like help with after treatment.
Meet Before Discharge
The client meets Jay by video before leaving rehab. Where helpful, the rehab clinician can also participate so relevant information can be shared directly, with the client’s consent.
Confirm Ongoing Support
Based on the client’s needs and existing supports, Jay and the client determine whether ongoing psychological and AOD support through Applied Psychology is appropriate.
Have the Next Appointment Already in Place
The first appointment with Applied Psychology is arranged before the client leaves treatment. Ongoing appointments can then be provided face-to-face on the Sunshine Coast or by video telehealth. Video telehealth is well established in psychological and AOD care. For example, one study involving more than 1,800 people receiving treatment for alcohol use disorder found that video-based treatment produced abstinence and other clinical outcomes similar to face-to-face care.
Make Sure the Link Happens
After the client returns home, Applied Psychology follows up to ensure the planned continuing care has commenced and addresses any problems with the connection.

Referral and intake forms are completed securely through Halaxy, our practice management platform. Information submitted is treated as confidential and handled in accordance with Applied Psychology’s Privacy Policy.
What My Rehab Link Does and Does Not Do
Applied Psychology has a specific role: supporting the transition from residential treatment into ongoing care in Australia.
What This Service Does

Replace the treatment or aftercare provided by the residential rehab

Provide medical or psychiatric treatment or prescribe medication

Manage every aspect of a client’s return home

Guarantee abstinence, recovery or any particular treatment outcome
Privacy, Consent and Clinical Responsibility
Privacy and Consent
Client information is shared only where it is relevant to the transition of care and with the client’s consent. Applied Psychology uses the minimum information necessary to support continuity of care. A full residential treatment file is not routinely required.
Clients may provide consent for relevant information to be shared:

from the rehab to Applied Psychology

back to the rehab where limited transition feedback is appropriate
Any feedback provided to the rehab relates to the transition itself, for example, whether continuing care has commenced, rather than the content of ongoing therapy.
Information Storage and Security
Information collected is stored within Applied Psychology’s Halaxy practice management system.
Halaxy states that Australian practice data is stored in Australia, protected by encryption at rest and in transit, and that it is bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Halaxy has also been formally certified to ISO/IEC 27001, the international standard for information security management.
Clinical Responsibility
While the client is in residential treatment, clinical responsibility remains with the treating rehab. After the client returns home, responsibility for ongoing psychological treatment rests with the Australian treating clinician or service.
Medical, psychiatric and prescribing decisions remain the responsibility of the appropriately qualified medical practitioner. Applied Psychology coordinates the transition between services but is not an emergency or crisis service.
